LOGIN to submit an update to the 2-1-1 Database online or download and submit the appropriate PROFILE FORMS.

 

ADD YOUR ORGANIZATION'S INFORMATION TO THE 2-1-1 DATABASE:

To be considered for inclusion in the 2-1-1 Santa Clara County database, please review the following instructions:

1. Verify that your organization and it's programs meet the 2-1-1 Santa Clara County INCLUSION POLICY criteria.
2. If you believe your organization meets the Inclusion Policy criteria, please complete and submit the required PROFILE FORMS.
3. Upon review and approvoal of your Profile forms your information will be added to the database.

Please note, multiple Profile forms (Agency, Site, and Service Profiles) may be required. See description below. Incomplete or missing forms will cause a signifigant delay in the review and processing of your application.

Agency Profile.pdf - Please fill out this form for your administrative office or headquarters.

Site Profile.pdf - Please fill out one form PER site or location within your organization offering a service. Multiple forms may be required depending on the number of sites at your agency. You do not need to fill out this form if your headquarters is the only site within your organization.

Service Profile.pdf - Please fill out one form PER service offered in your organization. Multiple forms may be required.
