Login to submit an update to the 2-1-1 Database using the Online Survey tool or download and submit our PROFILE FORMS.
To be considered for inclusion in the 2-1-1 Santa Clara County database, please review the following instructions:
1. Verify that your agency and programs meet our Inclusion
Policy criteria.
2. If you believe your organization meets our Inclusion Policy criteria, please complete and submit the Profile documents below. Read
the instructions carefully.
3. Upon review and approvoal of your Profile forms your information will be added to the database. Your designated 2-1-1 contact will receive notification of your approved record.
4. Profiles submitted with missing or incorrect information will result in signigant processing delays.
Please
note that, if applicable, multiple forms (Agency, Site, and Service Profiles)
may be required.

Site
Profile.pdf - Please fill out one form PER site
or location within your organization offering a service.
Multiple forms may be required depending on the number of
sites at your agency. You do not need to fill out this form
if your headquarters is the only site within your organization.
