Be sure to include the full name of your organization and your individual contact information when submitting an email. Not listed in the 211 database?

To be considered for inclusion in the 211 Santa Clara County database, please review the following instructions:

1. Verify that your organization and it's programs meet the 211 Santa Clara County INCLUSION POLICY criteria.

2. If you believe your organization meets the Inclusion Policy criteria, complete each of the required PROFILE FORMS available for download below.

3. Mail, fax or email completed forms to:

Upon review and approvoal of your Profile forms your information will be added to the database. Please note, multiple Profile Forms (Agency, Site, and Service) may be required. See description below. Incomplete or missing forms will cause a signifigant delay in the review and processing of your application.

Agency Profile Form - Use this form to submit information for your administrative office/headquarters.

Site Profile Form - Complete one (1) form for EACH site within your organization where a service is offered.

Service Profile Form - Complete one (1) form for EACH service or program offered by your organization.
